Important Information for Parents

PLEASE BE SURE TO READ THE LETTER FROM THE NURSE IN THE FIRST "GO HOME" FOLDER OF THE SCHOOL YEAR.

Rx Medications: 

• Must be in Rx bottle with correct information.

• Must have consent form signed by physician and parent/guardian with instructions.

Over-the-counter medications:

• Must be in original container.

• must have consent form signed by parent/guardian with instructions.

• Medications must be provided by student/parent.

Epi-Pens & Inhalers:

• Same instructions as Rx medications.

• Must have consent form signed by physician and parent/guardian with instructions.

***Notice to frequent volunteers***

Written documentation of a recent TB Test (within the last 3 years) must be on file in the nurse's office.

***PLEASE BE SURE TO FILL OUT EVERY EMERGENCY FORM COMPLETELY FOR EACH STUDENT ATTENDING SCHOOL. PLEASE UPDATE FORMS AS NEEDED IN  A TIMELY MANNER.***
